The MyPHIT application no longer runs at this address. The screen below
is a reproduction of the archived interface: its controls are disabled, nothing is submitted,
and no data is collected.
This was the Devise registration screen, and it is the most informative page in the backup. Alongside email, name, age and gender it asked for a role, and the recorded options — high-school, undergraduate, nursing, graduate and medical students, then residents, nurses, physicians and other health-care staff — show the audience the tool was written for. The password field carried the hint “Minimum password length: 6”.
