The MyPHIT application no longer runs at this address. The screen below is a reproduction of the archived interface: its controls are disabled, nothing is submitted, and no data is collected.

This was the Devise session screen. A returning participant entered the address they registered with, and the remember-me option kept the session across visits. The archived page also carried links to registration and to password recovery.
